Texas Wound Solutions is seeking a highly skilled and compassionate Nurse Practitioners/PAs to join our team, providing mobile in-home wound care services to patients. The ideal candidate will have advanced clinical expertise in wound care and a passion for delivering patient-centered care in a home-based setting. Hiring in Dallas, Fort Worth, Houston, Austin and San Antonio.
As a Mobile Wound Care Specialist, you will assess, diagnose, and manage various wound types, including surgical wounds, pressure ulcers, diabetic ulcers, and ostomy care needs. You will play a vital role in improving patient outcomes through evidence-based treatments, patient education, and ongoing monitoring.
Key Responsibilities
- Conduct comprehensive assessments of patients’ wounds and overall health status in their homes.
- Develop and implement individualized care plans tailored to each patient’s needs.
- Perform wound care treatments, including debridement, dressing changes, compression therapy, and ostomy management.
- Collaborate with other healthcare providers, including physicians, nurses, and specialists, to ensure optimal care.
- Educate patients and caregivers on wound care techniques, prevention strategies, and proper nutrition.
- Order and manage medical supplies and wound care products as needed.
- Document patient care and progress in the electronic medical record (EMR) in compliance with organizational and regulatory standards.
- Stay updated on best practices and innovations in wound care management.
Qualifications
- Current and unrestricted Nurse Practitioner (NP) license in Texas.
- Board Certification in Wound Care (preferred).
- Certification as a Family or Adult-Gerontology Nurse Practitioner (FNP-BC, AGNP-BC, etc.).
- Minimum of 2 years of clinical experience in wound care or a related field.
- Proficiency in electronic medical records (EMR) and mobile healthcare technology.
- Excellent communication and interpersonal skills.
- Valid driver’s license and reliable transportation for travel to patient homes.
Preferred Skills
- Experience in mobile healthcare or home health settings.
- Knowledge of chronic disease management related to wound healing.
- Ability to work independently while collaborating effectively with a multidisciplinary team.
